It speaks of how being a Christian makes a difference in being a physician. The philosophies of the authors made a colorful explanation in the book on how virtue ethics counts a lot. Faith, hope and charity were given emphasis since it is the three Christian virtues that medical practitioners should acquire in their profession. Many things in the book relate medicine and ethics which made it interesting especially if you are a believer of the Christian faith. The book is designed with faith so that physicians will dwell not only on medical but with spiritual healing as well. It has weakness in a sense that doctors who do not profess a Christian faith will surely goes against this book. In addition to this, there are some medical explanations that cannot be simply linked to oneââ¬â¢s belief in faith and virtues. Nevertheless majority of the book talks about ethical principles pertaining to morality, spirituality and virtue focused medical practices in health care. ââ¬Å"Faith points the way, hope sustains us on that way and charity is the ordering principle which deliberation, raising morality to the level of love (Pellegrino and Thomasma, 1996). The aforementioned virtues will give more meaning to medicine because it is an additional healing factor to the contemporary ethics of medicine. The authors also talks about how Christian physicians may not acquire these virtues but it is a must for them to have it included in their profession. There were also viewpoints in the book telling how faith, hope and charity in medical practices should be taught even to physicians who are non-believers of Christian faith. Cases of euthanasia, abortion and incompetent surgery were cited as an example to defend the importance of attributing morality and virtue ethics in patient treatment. Violent means violates the virtue of charity which for the book is really against the Christian way of medical practice. There are hypocrisies that can be found in the book since clashing of principle based and virtue based ethics were present. It serves as a guide on how physicians should act according to the Christian teaching and at the same time never sacrifices their profession as medical practitioners. For the book itself, moral judgment is an art. Moral life is designed to make the right decisions during medical circumstances. The physician in charge should incorporate discernment and right motivation in everything he or she does. The book is not for the most part concerned with the substance of ethical decisions but with the manner of arriving at them. It is focused merely on virtue ethics and has a Christian moral philosophy that looks upon St. Thomas Aquinas and St. Augustine. This ethical virtue can never be neglected and there have been considerable discussions of both moral theology and philosophy. Fundamentals of the book lie in the Christian understanding of the physicianââ¬â¢s dignity. It is overwhelming to notice how the authors cite the words of Pope John Paul II as examples to fully explain their points of view. Physicians who are great followers of Christianity and incorporate medication with a heart will surely enjoy the book and share it to their non-Christian colleagues. Shadows in Fifth Business à Incidences that occur in one's childhood tend to affect them possibly for the rest of the rest of their life.à This applies to the novel Fifth Business and the characters Dunstan Ramsey and Boy Staunton.à Throughout the lives of these characters Dunstan lives in the shadow of Boy due to feelings of guilt and responsibility as a result of one winter evening in the town of Deptford. à As Boy and Dunny grew up together they were each others best friends and also worst enemies, but they were on basicallyà equal terms in their childish trials of life.à This remained to be true until that winter day in Deptford when Mrs. Dempster was struck with a snowball that was meant for Dunny.à Since that point in time Dunny, being the more caring and excepting of responsibility for other peoples problems, of the two, has not lived up to par with the life of Boy Staunton. à Ever since the snowball accident Dunny has been preoccupied by worryingà over Mary Dempster, and now her son Paul.à At the age of sixteen the small town of Deptford becomes too much for Dunny to handle so he decides to drop out of secondary school andà join the Army.à Dunny needed a change in his life, something to get his mind off Mrs. Dempster and the guilt he felt for her.à Leading up to his departure to the War he never really saw much of Mary, mainly because Mr. Dempster told him to stay away, but also because every time him saw her he couldn't hold back feelings of guilt and remorse.à This troubled Dunny, much more then he would ever let on.à On the other hand, Boy was doing as well as ever, possibly due to the fact that he knew that much of the responsibility of Mary and Paul was securely on the shoulders of Dunny.à à Dunny knew this as well but it was too late to do much about it except leave. à While Dunny was away at war, Boy was still finishing school and in the process of stealing Dunny's girl while he was away.à Boy and Dunstan had been competing for Leola all their life up until when Dunny left.à This loss for Dunstan could have been avoided if in fact he had notà accepted responsibility for the snowball incident he wouldn't have had to leave and therefore could have kept Leola.

A comparison between The Woman In Black and The Signalman Essays A comparison between The Woman In Black and The Signalman Paper A comparison between The Woman In Black and The Signalman Paper Essay Topic: Literature The Black Monk When reading both ghost stories, The Woman in Black, and The Signalman, the text which I enjoyed and prefered was the Woman in Black. Susan Hill, the author of this novel creates a menacing and baleful atmosphere that can be rather unsettling for any reader. She also creates a presence of evil throughout the story, as it is portrayed through setting, characters, plot and descriptive language. Susan Hill sets the first scene in the first chapter with the description of the pleasant festive meal, which had taken place on Christmas Eve at Monks Piece. Arthur Kipps the narrator was fearful of his memories intermingling with the festivities, as his family were rather keen for him to tell a ghost story, I was trying to suppress my mounting unease, to hold back the rising flood of memory. Unfortunately no one in his family knew what he had been through earlier in his life, when he came to terms with the Woman in Black. The beginnings of both texts are completely different. When first reading, The Signalman, it appears that the signalman is in a remote location as it is in a cutting where a train passes through. The Signalman works alone, making his post a solitary one. His post was in a solitary and dismal place as ever I saw. So little sunlight ever found its way to this spot, that it had an earthy deadly smell; and so much cold wind rushed through it, that it struck chill to me, as if I had left the natural world. This last quote tells me that this place is not natural and that the narrator has entered an entirely different world. Both these two texts are written in the 1st person but Dickens uses his narrator in the Signalman to tell the story in the perfect tense. In, The Signalman, the narrator is not actually haunted by ghost so he cannot describe his emotions and feelings whereas in the Woman in Black the narrator, Arthur Kipps, experiences the hauntings of the Woman in Black so, is clearly able to express his feelings and emotions about his hauntings. When Susan Hill develops the narrators feelings, she uses long passages of description that is rather effective. She sets the scene and anxiety and foreboding has begun. Something sinister has happened to Kipps and we know that in chapter 2, A London Particular. The story of Kipps is about to unfold. He relates being called to attend an interview with Mr. Bentley and although it was many years ago he can remember the smallest details of that day travelling up to London. Susan Hill also uses different place names and weather conditions to create an atmosphere of evil, for example, the nine lives cause-way, the mist and the fog. In, The Signalman, Charles Dickens does not use much description, He was a dark sallow man, with a dark beard and rather heavy eyebrows. But it is more than we know of the visitor who, along with the signalman, remains nameless and he seems to have no apparent reason for his visit to the signalman and shows no earlier or previous relationship him. Charles Dickens leaves many things unknown. Firstly we know little about the signalmans past but what we do know doesnt seem to add up to this present situation. We know he is well-educated man but is strange as a signalman is a rather lowly post. We know that he ran wild in his youth, but we know not of the circumstances. However, both these narrators have something in common as they both look back on incidents. The haunting in, The Signalman, conveys irony and sadness for the signalman rather than trying to frighten the reader. Charles Dickens does this particularly well and if the text is read well you can sympathise with the signalman and what the ghost is doing to him. The author does this well in my opinion. In the nursery in, The Woman in Black, the woman in black has just got off the chair and passes Arthur Kipps. Susan Hill writes this to disturb and daunt you with the adjectives that she uses. The sound and images created in your head are meant to terrify you. Susan Hill builds up the atmosphere well and really does haunt you well because it is a more detailed account. In, The Signalman, we know nothing about the ghost, we dont know if it is a past victim of a train crash or any other murder or if it is there as a guardian for the signalman or just to warn the signalman of an upcoming accident. The irony of it is, is that the spectre finally forecasts the signalmans own death. Charles Dickens varies the pace of this short ghost story and I found this to be rather difficult to apprehend. To conclude I feel that, The Signalman, meets all criteria needed for a good ghost story. Its not, hair raising stuff but there is a physiological doubt in the readers mind on whether the signalman is going mental or what is happening is true. I think that for the time that it was written it would have been very disturbing literature to read. But todays standard it doesnt have the same appeal that other novels have. Though I am sure it will go down in history as a classic. In, The Woman in Black, Susan Hill builds suspense throughout the story line. The suspense is created mainly by hints and clues given by characters, and sightings of the ghost building up to a climax. For example, when Kips sees the Woman in black in the graveyard and also when at Mrs Drablows funeral. At the end of the novel there is another climax, an unexpected one, the death of Kipps wife and child. The narrator describes a peaceful, happy Sunday afternoon in summer. He describes a festive holiday air, stalls selling ice creams, small boats being rowed and a band playing. Arthur Kipps then goes on to tell of his child and wife enjoying a ride in the pony and trap. It was a happy scene depicting good when suddenly Kipps sees the ghost of Jennet Humfrye-everything changed-good changed to evil. The story sadly finishes with Stella and the baby dying instantly. Whenever her ghost is seen, a child dies but in this instant Kipps wife also dies. The character Jennet Humpfrye comes across as a very bitter woman as she cannot forgive her sister, Alice Drablow for the death of her son Nathaniel. She sees her sister as someone who robbed her of happiness and motherhood and would never allow her to forget it. Thus, she therefore took her revenge on those living locally by haunting them. Susan Hill has used the theme of evil in the novel as if there was another character. The evil makes the book effective. It is believeable as well because Arthur Kipps was a calm, rational man who underwent a great change due to the evil he encountered. The ending of the story is very dramatic and makes the reader feel that Kipps really doesnt want to say anymore and will be glad to put the past to rest. They asked for my story. Ihave told it Enough. This was why I preferred, The Woman in Black, as it enticed you into the novel.

Four American indie authors were present: Bella Andrà ©, Tina Folsom, Debra Holland and Shelley Adina (who is also Bellaââ¬â¢s copy editor) along with the representatives of a new European association, the EWWA.About the Womenââ¬â¢s Fiction FestivalBut before I get there, letââ¬â¢s talk a bit about the festival itself. It is organized essentially by three incredibly energetic women: Elizabeth Jennings (author and translator, and a member of the ALLi), Maria Paola Romeo and Mariateresa Cascino. It has been taking place every year for the past 11 years in the magical city of Matera (I spent most of my spare time taking pictures.The choice of the city, which might be surprising for a truly international conference, isnââ¬â¢t a coincidence. Matera is currently running for the ââ¬Å"2019 cultural capital of Europeâ⬠title, and thus supports and sponsors a lot of cultural events and conferences. Moreover, it is a city laden with history. It underwent an exceptional change over the past 50 years, and is now living in a mix of traditionalism and modernism. Process Writing for ESL and EFL Process writing is an approach to incorporating writing skills from the very beginning of the English learning process. It was developed by Gail Heald-Taylor in her book Whole Language Strategies for ESL Students. Process writing focuses on allowing students- especially young learners- to write with plenty of room left for error. Standard correction begins slowly, and children are encouraged to communicate through writing, despite limited understanding of structure. Process writing can also be used in an adult ESL/EFL setting to encourage learners to start working on their writing skills from a beginning level. If you are teaching adults, the first thing learners need to understand is that their writing skills will be well below their native language writing skills. This seems rather obvious, but adults are often hesitant to produce written or spoken work that is not up to the same level as their native language skills. By easing your students fears about producing sub-par written work, you can help encourage them to improve their writing abilities. Only mistakes made in grammar and vocabulary that has been covered up to the current point in time should be corrected. Process writing is all about the process of writing. Students are striving to come to terms with writing in English by writing in English. Allowing for mistakes and refining based on materials covered in class- instead of perfect English- will help students incorporate skills at a natural pace, and improve their understanding of materials discussed in class in a natural progression. Here is a short overview of how you can incorporate process writing into your students learning routine. Aim: Improve writing skills from beginning levels of EnglishActivity: Process writing - journalsLevel: Beginning to advancedMaterials Needed: Notebook for each student Outline Encourage learners to write in their journal at least a few times a week. Explain the idea of process writing, and how mistakes arent important at this stage. If you are teaching higher levels, you can vary this by stating that mistakes in grammar and syntax on material not yet covered arent importantà and that this will be a great way to review material covered in past levels. Students should write on the front side of each page only. Teachers will provide notes on the writing on the back. Remember to focus only on material covered in class when correctly student work. Start this activity by modeling the first journal entry as a class. Ask students to come up with various themes that could be covered in a journal (hobbies, work-related themes, observations of family and friends, etc.). Write these themes on the board. Ask each student to choose a theme and write a short journal entry based on this theme. If students do not know a particular vocabulary item, they should be encouraged to describe this item (for example, the thing which turns on the TV)à or draw the item. Collect the journals the first time in class and do a quick, superficial correction of each students journal. Ask students to rewrite their work based on your comments. After this first session, collect students workbooks once a week and correct only one piece of their writing. Ask students to rewrite this piece.

I attributed that void within me to my lack of a degree, and I want to prove to myself and to others that I deserve everything I have right now by backing it up with a degree. Education, for me, is a very important part of a personââ¬â¢s growth and development. That is why I strive not just to be able to provide my familyââ¬â¢s basic needs, but to also provide my kids the best education possible. Then it hit me, I had to set myself as an example to my children that, indeed, education is and will always be important. I had to earn my degree. During the course of my work, I can say that I am competent enough in my analytical skills; I can solve basic problems, find solutions, think logically and perform my tasks efficiently. But one problem I had is that I was weak at my written and oral communication skills because I have not undergone the proper training to develop them both; yes, I was able to apply them all throughout my career, but I was not able to produce formal letters without having to browse through guides that will help me produce my work. Information retrieval was never a problem because I am still young and my memory, although not as sharp as it used to be, is still very much dependable; regarding the utilization and collaboration of this data though, I have been able to manage, but I fear that it is not as efficient as I would like to be because I have not yet been able to hone my skills and tackle formal ways to address issues efficiently. When I started to work, my goal was simple: stay in this job so that in the long run, when I start my own family, I can be able to provide them with their basic needs such as a proper home, enough food, and good education. But no one hinders me from dreaming; as time passed I started to change these goals, I want to be better-off, now Iââ¬â¢m dreaming of gaining a high position someday, be able to pamper my wife and my children luxurious gifts, and basically live the good life. Once again I thought about my lack of education; these dreams became the stepping stones which motivated me pursue my education and realize these career goals that I once thought are just dreams. During my stay at the University of Phoenix, the two core courses that I enjoyed the most were public relations and international marketing. The course in public relations has helped me a lot with my abilities to interact with people, the proper ways of doing things ââ¬â how to address them, how to project myself in a way that it exhibits confidence and knowledge at the same time ââ¬â these kinds of things. The international marketing course has helped me understand the doââ¬â¢s and donââ¬â¢ts in markets in a global scale, as well as be able to predict trends and fads. Considering that I am in the beverage industry, these courses will pretty much help me as I interact with people, not just the customers but also the higher ââ¬âups in both the domestic and foreign markets. The other two subjects that got me interested most were Critical Thinking and Human Motivation. In the Critical Thinking course, I was able to hone my problem solving and =general data processing and it has helped me a lot professionally because of the nature of my work. The cut throat world of industry has become a little easier for me now that I have these skills. The other course, Human Motivation, has helped me with in interacting with people from all walks of life, and I believe that it has been and will be very helpful in my future endeavours. Human motivation, as I see it, does not box the people in, rather it helps them get ââ¬Å"out of the boxâ⬠by motivating them, not forcing them. My completion of this degree helps prove one of the most obvious facts in life ââ¬â that life is indeed a never ending journey. I will never learn everything there is to know, but this will not stop me from being curious, that is why I have started to crave for learning not just because it will help me develop myself into an educated, motivated individual, but it will also help bring out all that is best within me. For the next five years, hopefully I can further pursue higher learning and expand my career by getting a promotion. I feel that I now deserve any type of promotion that I will be able to achieve given that I can already back it up with my degree. If I will come across programs such as seminars, forums, and such discussions about critical thinking and human motivation, then I would gladly undertake it because I feel that there is still much to learn in these subjects. For me, human interaction is a skill that one can be good at but can never master, and I will, as I have always done, continue to work on these skills by interacting with my peers, my customers, and my superiors. The difference now, though, is that I have formal knowledge on how to tackle situations that I would not have otherwise known if I had not finished my education.

Ben Franklin: Early Life In his many careers as a printer, moralist, essayist, civic leader, scientist, inventor, statesman, diplomat, and philosopher, for later generations of Americans he became both a spokesman and a model for the national character. He was born in Boston, Massachusetts on Jan. 17, 1706, into a religious Puritan household. His father, Josiah, was a candlemaker and a skillful mechanic. His mother, Abiah Benââ¬â¢s parents raised thirteen children--the survivors of Josiahââ¬â¢s seventeen children by two wives (#1). Printer & Writer Franklin left school at ten years old when he was pressed into his father's trade. At twelve Ben was apprenticed to his half brother James, a printer of The New England Courant. He generally absorbed the values and philosophy of the English Enlightenment. At the age of 16, Franklin wrote some pieces for the Courant signed "Silence Dogood," in which he parodied the Boston authorities and society (#3). At one point James Franklin was imprisoned for his liberal statements, and Benjamin carried on the paper himself. Having thus learned to resist oppression, Benjamin refused to suffer his brother's own domineering qualities and in 1723 ran away to Philadelphia (#1). Soon Franklin found a job as a printer. After a year he went to England, where he became a master printer, sowed some wild oats, amazed the locals with his swimming feats, and lived among inspiring writers of London. By 1726 Franklin was tiring of London (#1). He considered becoming an itinerant teacher of swimming, but when a Quaker merchant by the name of Thomas Denham offered him a clerkship in his store in Philadelphia, he decided to return home (#5). Returning to Philadelphia in 1726, he soon owned a newspaper, the Pennsylvania Gazette, and began to print Poor Richard's Almanac. In the Pennsylvania Gazette, a citizen asked editor Franklin the following question: "If A found out that his neighbor B was sleeping with his wife, was he justified in telling B's wife, and persuading her to seek a little revenge with A?" The editor's response: "If an ass kicks me, should I kick him again? (#4)" His business expanded further when he contracted to do the public printing of the province, and established partnerships with printers in other colonies. He also operated a bookshop and became clerk of the Pennsylvania Assembly and postmaster of Philadelp... ...eek. These virtues include temperance, silence, order, resolution, frugality, industry, sincerity, justice, moderation, cleanliness, tranquillity, chastity, and humility. The Autobiography is not the full story of Franklinââ¬â¢s life. It terminates approximately in his fifty-third year, before he became Americaââ¬â¢s greatest diplomat. Poor Richard was an uneducated but experienced homespun philosopher, created and edited by Ben Franklin from 1732-1757. Although Poor Richard of the early almanacs was a dim-witted and foolish astronomer, a round character soon replaced him who was a rich source of prudent and clever aphorisms on the value of economy, hard work, and the simple life. Food Intake Three Days Food Intake Three Days Healthy eating is important to the human body. Healthy eating provides the body with nutrients and energy to help the body function properly. Nutrients provide the body energy to keep the brain cells active, the heart pumping, and the muscle flexible. Nutrients also help to build strong bones, enhance the circulatory system, and other bodily functions. Healthy dieting means to consume all food groups so the diet can have a variety of fruits, vegetables, grains, milk, meats, bean, and oils.Even when eating from all food groups individuals must eat in moderation controlling the fats, salts, and sugars to maintain a healthy diet (Richford, 2011). Upon enrolling in a Human Nutrition class the professor assigned an individual assignment to assess my eating habits through iprofile factoring my weight, height, age, meals, and daily activities. The assessment evaluates three days of eating breakfast, lunch, dinner, and snacks. The nutrients asses sed were the lipids, proteins, and carbohydrates to access my positives and negatives about healthy eating.I entered my daily intake for three days and what a discovery. I revealed that my protein intake was excellent 70%, my lipids were high by 11%, and my carbohydrates were 80% below the recommended DRI. The results shocked me because I am not a regular meat eater, but my protein intake was in excellent. The carbohydrates did surprise me at all because I love eating vegetables, sweets, and I consume very little to none to be honest. Because I am fully aware that I need to make a change to avoid further issues, here are some ways that I intend to improve my carbohydrates.First I will eat more vegetables consuming the recommended amount two cups per day, ensuring that the vegetables are different every week. Carbohydrates are the main energy source for cells and organs. The basic carbohydrate units are called monosaccharideââ¬â¢s, such as glucose, GA lactose, and fructose. Glucos e and glycogen are stored in the liver and when the body becomes. Carbohydrates help fuel the body and create sugar. Sugar causes the body to release insulin and stress hormones. Consuming too much carbohydrate leads to obesity, diabetes, and several more health problems.The groups of foods that are underrepresented in my diet are grains, fruits, milk, and vegetables. The groups of foods that are overrepresented are meat and beans and discretionary. I honestly do not have a good excuse as to why I did not consume the recommended number of servings for each of the food groups. I only have myself to blame and I believe that my reasoning is sad. I am busy with school and work, I like food. I eat what I want when I want it which is not healthy. Proteins are important for repair and tissue growth helps the body to make enzymes and hormones, as well as preserve lean muscle mass and is a source of energy.My daily protein recommendations 71 grams. Protein has many important functions in the body and is essential for growth and maintenance. Protein needs can easily be met by eating a variety of plant-based foods. Combining different protein sources in the same meal is not necessary. Sources of protein for vegetarians and vegans include beans, nuts, nut butters, peas, and soy products (tofu, tempeh, veggie burgers). Milk products and eggs are also good protein sources for lacto-ovo vegetarians. Iron sources for vegetarians include cereals, spinach, and kidney beans.Calcium is used for building bones and teeth and in maintaining bone strength. Sources of calcium for vegetarians and vegans include calcium-fortified soymilk, calcium-fortified breakfast cereals and orange juice, tofu made with calcium sulfate, and some dark-green leafy vegetables the amount of calcium that can be absorbed from these foods varies. Consuming enough plant foods to meet calcium needs may be unrealistic for many. Milk products are excellent calcium sources for lacto vegetarians. Dietary fiber is prominent in recommendations for prevention of both coronary heart disease and cancer.The National Cancer Institute gives primary emphasis to dietary fiber in its recommended food choices. A range of intake of 20 to 30 g/d is suggested for US adults. The National Heart, Lung, and Blood Institute, through its National Cholesterol Education Program, emphasize reduction in fat intake and compensatory energy replacement through the increased intake of other foods, including those with high fiber content. Too many proteins chronically hard of kidneys, such as progression of renal failure, gout and cancer.Excessive intake Carbohydrates make high risk of blood sugar and are heart disease. Winter Santiago Isn't Just a typical teenager. She Isn't typical at all, she barely ever goes to school, she only goes when she wants something. Her family life Is anything but typical, her father Is a big drug dealer with a lot of money and her mother Isn't even Like a mother to Winter, she's more Like a sister to her. Winter Is very self- absorbed; she only worries about herself, even when her mom got shot In the face Winter gets jealous because she thinks her mother's hogging all the attention.She really only cares about herself. She is very street smart though, after her father goes to prison, her mother becomes a crack head and her sisters go into care, Winter knew how to take care of herself, she knew what to do to survive on the streets without the help of her father. She's done horrible things to survive and live the life she knew, like robbing people and betraying her family. Winter wouldn't make a good best friend at all; I don't even think she would make a good friend.She betrayed all of her friends one way or another, either by hooking up with their man because he had what she needed, or betraying Simons when she got arrested because she was telling for Winter, Simons was ready to have her baby and even though she said no, Winter persisted that she go and steal for her, because she wanted something, she didn't care that Simons was having a baby, if anything she used it against her saying she would need the money that she's making stealing, for the baby, in the end Simons lost the baby, and in my opinion a lot of the blame should be on Winter.She may be a fun person to be around and go out with because she does know how to have fun, but I would never want her as a friend because she's not loyal and she is untrustworthy.She does have a of of maturing to do though, so I really shouldn't judge too harshly, you can tell by the end of the book, when she Is In prison, and goes to her mother's funeral, she's a different person, her views on life are totally different, and when she sees her younger sister doing the same things she was doing she wants to stop her and tell her everything that she had gone through, but she held back knowing that it would only make her sister angry and make Winter look Jealous.I think that going to prison changed Winter In the best way, she was no longer the elf-absorbed, conceited ââ¬Å"ghetto-princessâ⬠that she was before she had gone to prison, she actually grew up, and matured during her time In. Yet, just like any person who has been abusing his body regularly over a period of time, our planet has aged immensely over the past decades. Everyday, I open the newspaper and read about various environmental abuses, turning on the television shows me the effects of global warming on the planet. Global warming, La Nina, El Nino, no matter what we call the weather phenomenon the truth is that the earth we live on is sick. Gravely ill and asking for help to find a cure for what ails it. Afflicted with various illnesses for which there does not be a cure in sight, the real question is, can we collectively find a cure for all that ails our planet before it is too late? Sometimes, I look out my window and worry about what the next reaction of the planet will be to the constant abuse. Will my children still have a planet to call home when they grow up? Will there still be resources available to them in the future that will help them survive the constantly changing weather patterns? I feel like the past generations and the present have taken so many liberties that the earth has come to the point of no return, it may be too late to turn things around and preserve what is left for the future generations. Strange illnesses for which there does not seem to be a cure seem to be plaguing us. Perhaps, that is the after effect of all the poisons that permeate our air. God only knows that nobody ever intended for all of this to happen as we strive to improve our lives. It may not be too late to save the planet. The question remains though as to how much mankind is willing to change in order to preserve what is here and to help restore the natural balance that was intentionally disturbed by us. The planet is crying out for help. It is our job as a collective to make sure that we help it find a cure and save its life. After all, if this planet dies, so do we, for this is the only place we can call home.

Communication Skills in Nursing | Reflection This essay will present a reflective account of communication skills in practice whist undertaking assessment and history taking of two Intensive Care patients with a similar condition. It will endeavour to explore all aspects of non verbal and verbal communication styles and reflect upon these areas using Gibbs reflective cycle (1988). Scenario A Mrs James, 34, a passenger in a road traffic collision who was not wearing a seatbelt was thrown through the windscreen resulting in multiple facial wounds with extensive facial swelling which required her to be intubated and sedated. She currently has cervical spine immobilisation and is awaiting a secondary trauma CT. Mr James was also involved in the accident. Scenario B Mr James, 37, husband of Mrs James, the driver of the car, was wearing his seat belt. He had minor superficial facial wounds, fractured ribs and a fractured right arm. He is alert and orientated but currently breathless and requiring high oxygen concentrations. Patients who are admitted to Intensive Care are typically admitted due to serious ill health or trauma that may also have a potential to develop life threatening complications (Udwadia, 2005). These patients are usually unconscious, have limited movement and have sensation deprivation due to sedation and/or disease processes. These critical conditions rely upon modern technical support and invasive procedures for the purpose of monitoring and regulation of physiological functions. Having the ability to effectively communicate with patients, colleagues and their close relatives is a fundamental clinical skill in Intensive Care and central to a skilful nursing practice. Communication in Intensive Care is therefore of high importance (Elliot, 1999) to provide information and support to the critically ill patient in order to reduce their anxieties, stresses and preserve self identity, self esteem and reduce social isolation (Joà £o: 2009, Alasad: 2004, Newmarch:2006). Effective communic ation is the key to the collection of patient information, delivering quality of care and ensuring patient safety. Gaining a patients history is one of the most important skills in medicine and is a foundation for both the diagnosis and patient clinician relationship, and is increasingly being undertaken by nurses (Crumbie, 2006). Commonly a patient may be critically ill and therefore the ability to perform a timely assessment whilst being prepared to administer life saving treatment is crucial (Carr, 2005). Often the patient is transferred from a ward or department within the hospital where a comprehensive history has been taken with documentation of a full examination; investigations, working diagnosis and the appropriate treatment taken. However, the patients history may not have been collected on this admission if it was not appropriate to do so. Where available patients medical notes can provide essential information. In relation to the scenarios where the patient is breathless or the patient had a reduced conscious level and requires sedation and intubation, effective communication is restricted and obtaining a comprehensive history would be inappropriate and almost certainly unsafe (Carr, 2005). The Nursing Midwifery Council promotes the importance of keeping clear and accurate records within the Code: Standards of Conduct, performance and ethics for nurses and midwives (NMC, 2008). Therefore if taking a patients history is unsafe to do so, this required to be documented. Breathing is a fundamental life process that usually occurs without conscious thought and, for the healthy person is taken for granted (Booker, 2004). In Scenario A, Mrs Jamess arrived on Intensive care and was intubated following her facial wounds and localised swelling. Facial trauma by its self is not a life threatening injury, although it has often been accompanied with other injuries such as traumatic brain injury and complications such as airway obstruction. This may have been caused by further swelling, bleeding or bone structure damage (Parks, 2003). Without an artificial airway and ventilatory support Mrs James would have struggled to breathe adequately and the potential to become in respiratory arrest. Within scenario B, Mr James had suffered multiple rib fractures causing difficulty in expansion of his lungs. Fractured ribs are amongst the most frequent of injuries sustained to the chest, accounting for over half of the thoracic injuries from non-penetrating trauma (Middle ton, 2003). When ribs are fractured due to the nature and site of the injury there is potential for underlying organ contusions and damage. The consequence of having a flail chest is pain. Painful expansion of the chest would result in inadequate ventilation of the lungs resulting in hypoxia and retention of secretions and the inability to communicate effectively. These combined increase the risk of the patient developing a chest infection and possible respiratory failure and potential to require intubation (Middleton, 2003). A key component of Intensive Care is to provide patients and relatives with effective communication at all times to ensure that a holistic nursing approach is achieved. Intensive care nurses care for patients predominantly with respiratory failure and over the years have taken on an extended role. They are expected to examine a patient and interpret their findings and results (Booker, 2004). In these situations patient requires supportive treatments as soon as possible. Intensive Care nurse should have the ability and competence to carry out a physical assessment and collect the patients history in a systemic, professional and sensitive approach. Effective communication skills are one of the many essential skills involved in this role. As an Intensive Care nurse, introducing yourself to the patient as soon as possible would be the first step in the history taking process (Appendix A) and the physical assessment process. Whilst introducing yourself there is also the aim to gaining consent for the assessment where possible, in accordance with the Nursing and Midwifery Councils Code of Professional Conduct (NMC, 2008). Conducting a comprehensive clinical history is usually more helpful in making a provisional diagnosis than the physical examination (Ford, 2005). Within Intensive Care the Airway, Breathing, Circulation, Disability, Exposure/Examination (ABCDE) assessment process is widely used. It is essential for survival that the oxygen is delivered to blood cells and the oxygen cannot reach the lungs without a patent airway. With poor circulation, oxygen does not get transported away from the lungs to the cells (Carr, 2005). The ABCDE approach is a simple approach that all team members use and allows for rapid asses sment, continuity of care and the reduction of errors. Communication reflects our social world and helps us to construct it (Weinmann Giles et al 1988). Communication of information, messages, opinions and thoughts are transferred by different forms. Basic communication is achieved by speaking, sign language, body language touch and eye contact, as technology has developed communication has been achieved by media, such as emails, telephone and mobile technology (Aarti, 2010). There are two main ways of communication: Verbal and non verbal. Verbal communication is the simplest and quickest way of transferring information and interacting when face to face. It is usually a two way process where a message is sent, understood and feedback is given (Leigh, 2001). When effective communication is given, what the sender encodes is what the receiver decodes (Baron:2005, Zastrow:2001). Key verbal features of communication are made up of sounds, words, and language. Mr James was alert and orientated and had some ability to communicate; he was breathless due to painful fractured ribs which hindered his verbal communication. In order to help him to breath and communicate effectively, his pain must be controlled. Breathless patients may be only able to speak two or more words at a time, inhibiting conversation. The use of closed questions can allow breathless patients to communicate without exerting themselves. Closed questions such as is it painful when you breathe in? or is your breathing feeling worse? can be answered with non ver bal communication such as a shake or nod of the head. Taking a patients history in this way can be time consuming and it is essential that the clinician do not make assumptions on behalf of the patient (Ashworth, 1980). Alternatively, encouraging patients to use other forms of communication can aid the process. Non verbal communication involves physical aspects such as written or visual of communication. Sign language and symbols are also included in non-verbal communication. Non verbal communication can be considered as gestures, body language, writing, drawing, physiological cues, using communication devices, mouthing words, head nods, and touch (Happ et al:2000, Alasad:2004). Body language, posture and physical contact is a form of non verbal communication. Body language can convey vast amounts of information. Slouched posture, or folded arms and crossed legs can portray negative signals. Facial gestures and expressions and eye contact are all different cues of communication. Alt hough Mr. James could verbally communicate, being short of breath and in pain meant that he also needed to use both verbal and non verbal communication styles. A patients stay in Intensive Care can vary from days to months. Although this is a temporary situation and many patients will make a good recovery, the psychological impact may be longer lasting (MacAuley, 2010). When caring for the patient who may be unconscious or sedated and does not appear to be awake, hearing may be one of the last senses to fade when they become unconscious (Leigh, 2000). Sedation is used in Intensive Care units to enable patients to be tolerable of ventilation. It aims to allow comfort and synchrony between the patient and ventilator. Poor sedation can lead to ventilator asynchrony, patient stress and anxiety, and an increased risk of self extubation and hypoxia. Over sedation can lead to ventilator associated pneumonias, cardiac instability and prolonged ventilation and Intensive Care delirium. Derlerium can be distressing for both the relatives andthe patient, who may have some recolection after the deleium epsiode (Mclafferty, 2007). Delirium is found to be a predictor of death in Intensive Care patients (Page, 2008). Every day a patient spends in delirium has been associated with a 20% increase risk of intensive care bed days and a 10% increased risk of morbidity. The single most profound risk factor for delirium in Intensive Care is sedation (Page, 2008). Within this stage of sedation or delirium it is impossible to know what the patients have heard, understood or precessed. Ashworth (1980) recognised that nurses often failed to communicate with unconscious patients on the basis that they were unable to respond. Although, research (Lawrence, 1995) indicates that patients who are unconscious could hear and understand conversations around them and respond emotionally to verbal communication however could not respond physically. This emphasises the importance and the need for communication remains (Leigh, 2001). Neurological status would unavoidably have an effect on Mrs Jamess capacity to communicate in a usual way. It is therefore im portant to provide Mrs James with all information necessary to reduce her stress and anxieties via the different forms of communication. For the unconscious patient, both verbal communication and non verbal communication are of importance, verbal communication and touch being the most appropriate. There are two forms of touch (Aarti, 2010), firstly a task orientated touch when a patient is being moved, washed or having a dressing changed and secondly a caring touch holding Mrs James hand to explain where she was and why she was there is an example of this. This would enhance communication when informing and reassuring Mrs James that her husband was alive and doing well. Nurses may initially find the process of talking to an unconscious patient embarrassing, pointless or of low importance as it is a one way conversation (Ashworth, 1980) however as previously mentioned researched shows patients have the ability to hear. Barriers to communication may be caused by physical inabilities from the patients however there are many types of other communication barriers. A barrier of communication is where there is a breakdown in the communication process. This could happen if the message was not encoded or decoded as it should have been (Baron, 2005). If a patient is under sedation, delirious or hard of hearing verbal communication could be misinterpreted. However there could also be barriers in the transfer of communication process as the Intensive Care environment in itself can cause communication barriers. Intensive Care can be noisy environment (Newmarch, 2006). Other barriers can simply include language barriers, fatigue, stress, distractions and jargon. Communication aids can promote effective communication between patient and clinician. Pen and paper is the simplest form of non verbal communication for those with adequate strength (Newmarch, 2006). Weakness of patients can affect the movement of hands and arms making gestures and handwriting frustration and diff icult. Patients may also be attached to monitors and infusions resulting in restricted movements which can lead to patients feeling trapped and disturbed (Ashworth, 1980). MacAulay (2010) mentions that Intensive Care nurses are highly skilled at anticipating the communication needs of patients who are trying to communicate but find the interpretation of their communication time consuming and difficult. The University of Dundee (ICU-Talk, 2010) conducted a three year multi disciplinary study research project to develop and evaluate a computer based communication aid specifically designed for Intensive Care patients. The trial is currently ongoing, however this may become a breakthrough in quick and effective patient clinical and patient relative communication in future care. This assignment has explored communication within Intensive Care and reflected upon previous experiences. Communication involves both verbal and non verbal communication in order to communicate effectively in all situations. Researching this topic has highlighted areas in Intensive Care nursing which may be overlooked, for example ventilator alarms and general noise within a unit may feel like a normal environment for the clinians however for patients and relatives this may cause considerable amounts of concern. Simply giving explanations for such alarms will easily alleviate concerns and provide reassurance. From overall research (Alasad: 2004, Leigh: 2001, MacAuley, 2010: Craig, 2007) Intensive Care nurses believed communication with critically ill patients was an important part of their role however disappointedly some nurses perceived this as time consuming or of low importance when the conversation was one way (Ashworth, 1980). Further education within Intensive Care may be requ ired to improve communication and highlight the importance of communication at all times. Communication is key to ensuring patients receive quality high standard care from a multidisciplinary team, where all members appreciate the skills and contributions that others offer, to improve patients care and the overall patient experience. Appendix 1, The History Taking Process: The questions are the key to a good interview. You need to use a mix of open ended questions and close ended questions. Open ended questions leave the door open for the patient to tell you more. Questions like when it this problem start?, have you had any recent health problems?, and can you show me where it hurts? are open ended. The patient feels free to provide additional information. While questions like does it hurt here?, did you have this pain yesterday?, and have you had the flu in the past month? are close ended. Close ended questions seek very specific, often yes or no responses from the patient and dont encourage the patient to provide any additional information.
